COVID-19 Pre-Screening Requirements
Entry into the Bellevue Club is permitted for members who display a temperature of 100.3 or lower upon check in, do not display symptoms of COVID-19, and have completed the waiver below.
All members are required to self screen for COVID-19 symptoms prior to entering. If you answered ‘yes’ to any of the below questions, we respectfully cannot allow you entry into the Bellevue Club.
No-contact temperature checks are administered at the Club entrance. Entry will not be permitted if you, or anyone with you, display a temperature of 100.4 or higher.
WAIVER FOR ENTRY
In consideration to the Club opening in accordance with current regulations relating to COVID-19, I acknowledge the contagious nature of COVID-19 and assume the risks associated with any exposure of this. It is my responsibility to warn my family and guests of the risks. I will not enter the Club with any flu-like symptoms and agree to take all reasonable steps to prevent my family and guests from entering the Club with any flu-like symptoms.
I voluntarily assume the risk that those on my membership account, including but not limited to myself, spouse/significant other, dependents or my guests, may be exposed to or infected by COVID-19 by attending the Bellevue Club and that such exposure or infection may result in personal injury, illness, disability, or death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Club may occur and I acknowledge that Bellevue Club cannot control or monitor the actions of every individual who enters the Club, and it takes only one infected person to spread contagion.
I waive any claim against the Bellevue Club and release the Club from liability, and I accept sole responsibility of COVID-19 exposure to anyone entering on my membership account or that we may experience or incur in connection with use of the Bellevue Club or participation in the Bellevue Club programming. I also agree that I will indemnify the Club for any damages, costs or legal fees resulting from Claims made by my spouse/significant other, dependents, or our guests. This release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Club program.
